Job summary

Avenues Recovery Center in Baton Rouge, LA is hiring Case Managers to join our network of rehab centers. You will advocate for clients, initiate intake-based case management, and ensure insurance coverage remains active throughout treatment.

You will coordinate housing, financial and legal resources and help plan discharge with ongoing support. Ideal candidates have a HS diploma or GED, at least one year in the treatment field, and a compassionate, proactive approach.

Qualifications

• High School diploma or GED (preferred).
• Minimum 1 year experience in treatment industry preferred.
• If in recovery, a minimum one-year sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Warm, empathetic, respectable personality.

Responsibilities

• Advocate for clients and address external barriers to treatment.
• Initiate case management upon intake and identify pressing needs.
• Oversee client accounts; ensure insurance remains active during treatment.
• Provide ongoing case management throughout treatment duration.
• Arrange housing, financial aid, legal aid, and vocational training as needed for post-discharge.
• Facilitate discharge planning including continuing care and sober support.
