WestCare Foundation is seeking a dedicated Case Manager for the STOP Program in Fresno, CA. This role handles a large caseload, ensuring thorough documentation and coordination of services across programs.

The position requires a high school diploma (or equivalent) with two years in criminal justice settings, plus a valid driver’s license and CDCR clearance. CPR/First Aid and annual TB tests are also required.

Formación

• Two+ years experience with criminal justice population.
• Case management experience preferred.
• High School diploma or equivalent required.
• Valid driver’s license and ability to obtain WestCare vehicle insurance.
• Must obtain and maintain CDCR clearance.
• CPR/First Aid certifications and annual TB test.

Responsabilidades

• Case management of a large caseload of participants released from prison substance abuse programs.
• Heavy documentation.
• Coordination of services across multiple agencies.
• Travel to Community Based Providers in a personal vehicle.
• In-prison presentations and parole collaboration.
• Perform all duties aligned with WestCare’s mission and guidelines.
