Job summary

Encompass Health Corporation in Murrells Inlet, SC is seeking a Case Manager to coordinate patient care from admission to post-discharge. You will work with an interdisciplinary team to develop and monitor individualized care plans, ensuring timely and appropriate discharges and patient outcomes.

You will navigate payor requirements, manage resources, and educate patients and families about rehabilitation options while upholding regulatory standards and delivering compassionate, patient-centered

Qualifications

• Active professional license in the relevant discipline (RN, social work, OT, PT, etc.).
• CCM® or ACM™ certification required or must be earned within two years.

Responsibilities

• Work with interdisciplinary team, guiding treatment plans.
• Coordinate tentative discharge plan and contingency plans.
• Plan and execute patient discharge experience.
• Monitor patient experience: quality, timeliness, payors.
• Facilitate weekly team conferences and modify treatment plans.
• Complete case management addendums and documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan actions.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission.
• Assess goals and complete case management addendum within 48 hours.
• Educate patient/family on rehab and Case Manager role; establish communication plan.
• Schedule and facilitate family conferences.
• Assist patient with resources to avoid discharge delays.
• Monitor compliance with orthotics/prosthetics ordering.
• Make referrals to post-discharge providers.
• Ensure accuracy of discharge and payor information.
• Participate in utilization review: data collection and trend review.
• Participate in on-call schedule.
