Case Manager – SUD Treatment
Location: Concord, MA (NEW Facility!)
Schedule: Part‑Time (may include occasional evening or weekend hours)
Pay Rate: $25–30 per hour
Pyramid Healthcare is a nationally recognized provider of behavioral health services, offering compassionate, evidence‑based care to individuals with substance use and co‑occurring disorders. We are opening our newest treatment center in Concord, MA, focused on Substance Use Disorder (SUD) treatment for adults.
Position Summary
We are seeking a dedicated and organized Case Manager to support clients and families through the full continuum of care. This role is essential to client success, providing case management services, facilitating communication with referral sources and external providers, coordinating aftercare planning, and supporting overall engagement in treatment.
Key Responsibilities
- Coordinate client care activities, including appointments, schedules, and treatment planning
- Communicate with referral sources and outside providers to ensure continuity of care
- Support the admissions process by verifying documentation, scanning records, and initiating external communication as needed
- Conduct orientations with clients and families in partnership with the primary therapist
- Provide case management services, including coordination of transportation, facilitation of case management sessions, and addressing legal/employment‑related needs
- Facilitate educational or skill‑based groups to support clinical services
- Develop and document comprehensive aftercare plans in collaboration with the treatment team and business development staff
- Provide aftercare documentation to relevant external providers and ensure follow‑through
- Maintain accurate, timely documentation in the electronic clinical record system (Carelogic)
- Participate in treatment team meetings and maintain collaborative communication with counselors and staff
- Ensure compliance with all confidentiality, licensing, and program standards
- Complete required trainings and perform other duties as assigned
Qualifications
- Education: Bachelor’s degree in a related field required; Master’s degree preferred
- Experience: Minimum of 2 years of case management experience preferred, ideally in behavioral health or substance use treatment
- Skills: Strong organizational, communication, and interpersonal skills; ability to manage multiple tasks and meet deadlines; knowledge of community resources and referral processes
- Requirements: Ability to maintain professionalism, confidentiality, and accurate documentation
