Job summary

Community Solutions Inc. (CSI) seeks a dedicated Case Manager to support our Day Reporting Centers in Santa Maria, CA.

You will conduct needs assessments, support client progress, and document plans and data in CSI systems to help parolees achieve self-sufficiency through vocational training and personal development. You will participate in CPR meetings, coordinate with employment specialists, and facilitate group and individual sessions while adhering to CSI policies and ethical guidelines.

Qualifications

• Bachelor's degree with one year in case management or related field.
• Strong decision-making, organizational, and interpersonal skills.
• Experience with assessments and facilitation is a plus.
• Experience using Microsoft Office and working in a team.

Responsibilities

• Conduct needs assessments within 7 days of intake and record data in CSI systems.
• Log into funder database within 5 days of client advancement.
• Participate in CPR meetings; present assessments and ISP updates.
• Provide and request peer support for client issues.
• Create program goals within 30 days of activation; bi-weekly reviews.
