Job Details: Job Location: Sheakley - Cincinnati, OH, Salary Range: $20.19 - $25.00Hourly

What You'll Do

Build Meaningful Relationships Develop strong, supportive relationships with youth, families, and community partners. Advocate for clients and empower them to achieve their personal goals. Serve as a trusted guide throughout the treatment and transition process.

Connect Clients to Resources Help individuals identify and access community supports, services, and opportunities. Partner with schools, employers, healthcare providers, and community organizations. Remove barriers that impact education, employment, and successful community living.

Keep Clients Moving Toward Success Assess client needs and help determine the most appropriate level of care. Coordinate individualized service plans, treatment goals, and discharge planning. Support clients in developing daily living skills, independence, and self-sufficiency. Assist with crisis intervention, stabilization, and ongoing support when challenges arise.

Keep Care Organized Maintain accurate documentation and client records. Complete progress notes and service documentation in a timely manner. Ensure compliance with agency standards and regulatory requirements. Work closely with the treatment team to coordinate quality care and positive outcomes.

Support Long-Term Success Begin discharge planning from day one. Help clients successfully transition back into their communities. Provide follow-up support after discharge to encourage continued success and stability.

Schedule & Travel
