Mercyhealth is seeking a Case Manager/Care Coordinator for Older Adult Care in Rockford, IL. This full-time role conducts assessments in client homes or facilities, determines CCP eligibility, develops care plans, and connects clients with needed services under IDOA CCP guidelines.

The position requires a Bachelor’s degree in social sciences or related field, with 1 year of elderly services experience, or an LPN/RN with equivalent experience.

Qualifications

• Bachelor's degree in social science, social work, or related field.
• One year of experience with provision of services for the elderly, may replace one year of college education up to and including four years of experience; or LPN or RN.
• 1 year experience in needs assessment and care planning.
• BLS certification within 90 days; IL Department on Aging Care Coordinator Certification.

Responsibilities

• Performs comprehensive assessments in home or health care facility to assess need for community services.
• Authorize CCP services and coordinate referrals to other benefits/services.
• Complete Choices for Care assessments for Nursing Home or Supportive Living Facility placement.
• Complete Medicaid applications when indicated.

Case Manager, Older Adult Care - Full Time - Mercyhealth Logo### Case Manager, Older Adult Care - Full TimeApply: Mercyhealth Physician Clinic-Rockton - Rockford, IL: Full time: Posted Today: 2026-02781**OVERVIEW**The Case Manager/Care Coordinator works under the program rules and guidelines of the Illinois Department on Aging (IDOA) Community Care Program (CCP) as the Care Coordination Unit (CCU) for Boone & Winnebago Counties.The Care Coordinator conducts comprehensive needs assessments in the participant’s home or health care facility, determines eligibility for CCP, develops a care plan to meet the participants’ and caregivers’ needs, and assists in gaining access to and receiving needed services.**SCHEDULE:** 40-hours per week- Weekend on-call duties every 2-3 times per year based on staffing availability. **SHIFT:** Days**ESSENTIAL DUTIES AND RESPONSIBILITIES*** Performs comprehensive assessments in the participant’s home or health care facility to assess need for community services.* Demonstrates ability to authorize CCP services and coordinate referrals to other benefits/services available to the participant.* Completes Choices for Care assessments for Nursing Home or Supportive Living Facility placement* Demonstrates knowledge of the various types of assessments, billing codes and documentation required.* Accurately completes Medicaid applications when indicated.* Advocates, intervenes and follows-up in a timely manner to ensure services remain in place and are appropriate to the changing needs of the participant.* Documents, meets established timelines, and completes follow-up and assessments on critical events.* Responds to participant/family inquires and phone calls in a timely manner.* Completes annual educational requirements for IDOA and Mercyhealth.**EDUCATION AND/OR EXPERIENCE*** Bachelor's degree in social science, social work, or related field* One year of experience with provision of services for the elderly, may replace one year of college education up to and including four years of experience; **or*** LPN or RN**And*** 1 year experience in needs assessment and care planning**CERTIFICATION/LICENSURE*** BLS certification required within 90 days* Successfully complete IL Department on Aging Care Coordinator Certification* Maintain a valid driver’s license and reliable transportation with proof of vehicle insurance when requested.* Have and maintain an acceptable driving record per Mercy’s HR-01 Fleet Safety policy.**PAY RANGE:**$19.14 - $30.63Mercyhealth is an Equal Opportunity Employer.
