Oak Plains Academy, a UHS company, seeks a dynamic Case Manager (MSW) to join a multidisciplinary team serving adolescents in a structured residential program near Nashville, TN.

The Case Manager will provide quality case management, admission support, and utilization review, collaborating with families and care providers while upholding treatment philosophy and trauma-informed care. Minimum MSW and licensure eligibility required; 1–3 years of psychiatric behavioral health experience preferred.

Qualifications

• Master’s degree in Social Work required.
• Licensed, LPC, LCSW or LMFT or eligibility for licensure track in social work or related field acceptable.
• 1–3 years of psychiatric behavioral health experience required; 3–5 years preferred.
• Valid Driver's License.

Responsibilities

• Provide quality case management, admission support, utilization review activities and services to residents, their families and care providers.
• Serve as a member of the treatment team supporting the organization’s treatment program and philosophy.
• Ensure delivery of quality treatment to residents and families.
• Demonstrate knowledge of case management, discharge planning, age-specific growth and development; crisis and behavior management; individual and family therapy theory practice.

Oak Plains Academy is a comprehensive and highly structured 98-bed residential treatment program offering psychiatric residential treatment services to adolescents, struggling with emotional and behavioral issues. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional team of psychiatrists, psychologists, social workers, therapists, nurses, and teachers serve hundreds of children and adolescents annually with the common goal of treating and healing trauma and building resiliency in the residents we serve. Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy campus features a full-size gymnasium, dining hall, separate academic building, and four dormitories. Job Summary
