Job summary

Horizon Behavioral Heal in Lynchburg, VA, is seeking a full-time Case Manager to support individuals at risk of mental health and/or substance use issues. Responsibilities include assessment, monitoring, linking to services, counseling, education, and advocacy, with potential work for priority population women with substance use disorders.

A valid driver’s license and travel to Horizon sites may be required.

Qualifications

• Bachelor’s degree in a human services field.
• Minimum of 1 year of relevant mental health, developmental disability, and/or substance use work experience.

Responsibilities

• Promote and support individuals at risk of developing mental health and/or substance use issues.
• Assess, monitor, and link to services; provide supportive counseling, education, and advocacy.
• Coordinate service planning, crisis intervention, and discharge planning; ensure access to community resources.
