Job summary

Fernbrook Family Center is seeking a compassionate Case Manager / Mental Health Practitioner (MHP) to deliver targeted mental health services to children, adults, and families. This role requires coordinating with QMHP supervision and supporting clients in accessing medical, educational, social, vocational, and financial resources.

Responsibilities include home-based services, documentation, referrals, and collaboration with community providers.

Qualifications

• Bachelor’s degree in a behavioral science field or equivalent experience as defined by Minnesota statutes.
• Valid driver’s license, reliable transportation, and acceptable driving record.
• Ability to pass a background check.

Responsibilities
