Job summary

Fernbrook Family Center is seeking Mental Health Practitioners to provide skills-based mental health services to children, adults, and families in home and community settings. The role supports daily functioning and resilience under QMHP supervision.

The position offers flexible hours with both salary and hourly options, and requires travel to client homes and Fernbrook locations in Milaca and Onamia. A strong emphasis on documentation, supervision, and ongoing education is essential.

Qualifications

• Reliable transportation and valid driver's license.
• Background check in accordance with agency and DHS requirements.
• Must meet one of the Minnesota qualification pathways for MH Practitioner or Clinical Trainee.

Responsibilities

• Provide skills-based mental health services to individuals and families in home and community settings.
• Meet required productivity benchmarks and weekly billable expectations.
• Use culturally responsive strategies to address client needs.
• Complete documentation to agency standards with quality and timeliness.
• Seek supervision and guidance as needed.
• Attend staff meetings to support professional development.
• Complete 30 hours of MH-related continuing education every two years.
• Coordinate care with Fernbrook staff and external providers.
• Uphold Fernbrook Family Center’s mission and values, following all policies.
