TPEG, Inc. is seeking a Case Manager to oversee intake, progress notes, discharge planning, and coordination of social, occupational, and educational resources for patients. The role requires collaboration with a treatment team to ensure smooth progression through the continuum of care.

Responsibilities include admission, assessment, treatment planning, monitoring progress, and post-care coordination. The position also involves staff development and occasional travel as needed to support clients

Qualificações

• Education: Bachelor's Degree in Psychology, Social Work, or related field.
• Two years’ experience in a behavioral health care setting.
• Strong leadership, communication, and organizational skills.
• Ability to work independently and as part of a team.

Responsabilidades

• Admits new patients by completing the intake process and orientation.
• Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment.
• Monitors cases, documents progress, and coordinates services.
• Communicates progress in weekly interdisciplinary meetings and with families.
• Prepares discharge plans and coordinates post-discharge requirements.
• Supports staff development and training to improve treatment results.
