PACS is seeking a licensed LVN/LPN professional in the United States to provide coordinated patient care with emphasis on cost-effective outcomes. The role requires an LVN/LPN license, RN preferred, and focuses on assessments, care planning, and collaboration with care teams.

The position offers a competitive salary range between 85,000 and 100,000 USD per year, with opportunities to contribute to care quality and resident safety in a high-volume environment.

Qualifications

• Bachelor’s degree in nursing or social work is required.
• Two years of clinical nursing experience preferred.
• CCM credential a plus; ongoing continuing education/licensing required.

Responsabilités

• Communicate plans of care with residents and families.
• Attend and participate in morning meetings to coordinate care.
• Answer questions about care, treatment plans, and illness progression.
• Monitor and adjust residents' statuses based on needs.
• Prioritize daily work based on resident needs and discharges.
• Complete required documentation.
• Coordinate care with residents, providers, facilities, and third-party payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Interact professionally with residents, families, staff and partners.
• Maintain timely charting and paperwork.

Formation

Bachelor’s Degree in Nursing or Social Work

Two years of clinical nursing experience

Certified Case Manager (CCM) a plus

Continued education/licensing

Good standing with Department of Public Health
