Job summary

Mount Olivet Rolling Acres in Chanhassen, MN, is seeking a Case Manager to oversee the case management team for the Hennepin County DD/CADI program. The role includes daily supervision, staff training, and delivering contracted case management to a partial load, with an emphasis on person-centered services.

The position offers 40-hour work weeks, daytime hours, a competitive annual salary of $70,800 (DOQ), and a comprehensive benefits package.

Qualifications

• Bachelor’s degree from an accredited college or university with major in social work, psychology, special education, nursing, sociology or closely related field
• Three years of experience with persons with developmental disabilities/related condition, mental health and/or aging population
• Desirable: BA/BS degree in Social Work, Psychology, Nursing or Human Services related degree, and qualify as a QDDP/QIDP
• Social Work Licensure
• Minimum of one year experience as a case manager at Mount Olivet Rolling Acres
• Proven leadership and supervision experience
• Must understand the person-centered process
• Agreement with continuous quality improvement process and mission/values
• Must pass Department of Human Services background check and maintain “qualified” status
• Must not be excluded from working in government health programs by the Office of Inspector General
• If using own vehicle for company business, must have a current valid driver’s license and must have insurance as required by state law
• Must be able to successfully complete company’s physical demands document

Job Summary:

Responsible for the overall management and functioning of multiple case managers. Responsible for daily supervision and training staff. In addition to providing contracted case management services to a partial case load. This position is for the Hennepin County DD/CADI team.

Mount Olivet Rolling Acres – MORA – is a diverse and inclusive nonprofit that has supported adults, seniors and children with developmental disabilities, behavioral health needs as well as age-related and chronic medical conditions for over 50 years in the Twin Cities metro. Due to continued growth in our Case Management department, we are seeking a Case Manager for clients in Hennepin County. Office location is in Chanhassen.

An array of services: Adult Day Services, Contracted Case Management, Community Supports, over 30 Residential Homes, Respite, Therapeutic Recreation, Training for Guardians & Families, and a Discover Summer Daytime Programming for Youth with Disabilities. Our Metro Crisis Coordination Program (MCCP) offers a safety net of crucial and cost-effective services for adults and children.

Schedule: 40 hours, M-F daytime/business hours.

Annual Salary: $70,800 annual salary , DOQ. Plus a strong benefit package
