Position Summary

The Case Manager is responsible for coordinating and overseeing services for individuals receiving support through True Care Services. This position works closely with clients, families, guardians, care coordinators, direct support staff, healthcare professionals, and other members of the individual's support team.

The ideal candidate is organized, communicates effectively, understands person-centered services, and is comfortable balancing client needs, staff coordination, documentation, and regulatory requirements.

Key Responsibilities
  • Coordinate and oversee services for an assigned caseload of individuals.
  • Maintain regular communication with clients, guardians, families, care coordinators, staff, and other members of the support team.
  • Ensure services are delivered according to each individual's approved support plan and goals.
  • Monitor client progress and assist with identifying changing needs.
  • Conduct client visits and follow up on concerns regarding services, health, safety, or quality of care.
  • Coordinate with Direct Services Providers and other staff to ensure appropriate client coverage and continuity of services.
  • Review staff documentation, progress notes, service records, and other required documentation for accuracy and completion.
  • Assist with developing and implementing person-centered goals and strategies.
  • Participate in team meetings, planning meetings, and other client-related meetings.
  • Assist with client intake, transitions, and service coordination.
  • Ensure required client records and documentation are completed and maintained according to company and state requirements.
  • Report and document incidents, concerns, or changes in client condition according to company policy.
  • Assist with resolving client, guardian, and staff concerns in a professional and timely manner.
  • Promote client choice, independence, dignity, community participation, and quality of life.
  • Maintain confidentiality and comply with HIPAA and company policies.
  • Work collaboratively with leadership to maintain high-quality services.
Qualifications
Required
  • Must be at least 18 years of age.
  • High school diploma or GED.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the Anchorage area as needed.
  • Ability to pass required criminal background checks.
  • Strong written and verbal communication skills.
  • Strong organizational and time-management skills.
  • Ability to manage multiple clients, deadlines, and priorities.
  • Basic computer skills and ability to learn electronic documentation systems.
  • Ability to work independently while communicating effectively with a team.
Preferred
  • Previous experience in case management, human services, behavioral health, healthcare, social services, or developmental disability services.
  • Experience working with individuals with intellectual or developmental disabilities.
  • Knowledge of Alaska Medicaid waiver or Home and Community-Based Services (HCBS).
  • Experience working with guardians, care coordinators, and interdisciplinary teams.
  • Experience supervising or coordinating Direct Services Providers.
  • CPR/First Aid certification or willingness to obtain required certifications.
Schedule
  • Full-Time
  • Primarily Monday through Friday
  • Some schedule flexibility may be required based on client needs, meetings, emergencies, or other service-related responsibilities.
  • Local travel throughout the Anchorage area may be required.
Compensation

Starting at $22.00 per hour, DOE

Starting compensation may vary based on relevant education, case management experience, developmental disability experience, and other applicable qualifications.
