Encore Community Services in Midtown Manhattan is seeking a Case Manager to deliver in-home case management and supportive counseling to older adults through EISEP, helping them age in place.

You will perform assessments, develop care plans, coordinate services with homebound meal and homecare agencies, and advocate to ensure needs are met. This full-time role offers benefits and professional growth within a mission-driven nonprofit.

Qualificações

• Master’s degree in social work or related field preferred.
• 3+ years of professional social work with elderly population.
• Experience with Expanded In-Home Services for the Elderly (EISEP) case management a plus.
• Familiarity with entitlement and benefits procedures for elderly clients.
• Ability to visit seniors at home and work in office, field, and telecommuting.
• Bilingual preferred (Spanish, Mandarin or Cantonese).

Responsabilidades

• Perform in-home assessments and develop care plans for homebound older adults.
• Provide ongoing case management, referrals, advocacy, and crisis intervention.
• Maintain records in the client database and case files.
• Coordinate referrals with homebound meal and homecare agencies.
• Ensure compliance with regulatory and program requirements and timely visits.
• Attend required trainings including DFTA training.
• Perform additional duties as assigned by supervisor and program director.
