Catholic Charities Brooklyn and Queens seeks a Case Manager III to provide in-home assessment and care planning for homebound elderly clients in the community. You will develop and implement care plans with clients and families to help them live safely at home.
Responsibilities include crisis intervention, intake assessments, and ensuring timely DFWA assessments and reporting. Travel to client homes and training sites may be required, with a focus on compassionate, comprehensive support.
For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities.
