Job summary

Mercy Housing is seeking a Case Manager II in San Francisco to work with residents to develop individualized plans and assess ongoing needs. You will help residents identify skills to promote growth and mentor junior staff.

The role is on-site, with a focus on coordinated services, resident empowerment, and program delivery in a busy housing environment. A relevant degree and experience are required.

Qualifications

  • Bachelor's Degree in Social Science or related field required.
  • Three (3) years of experience working with homeless or low- and mixed-income populations required.
  • Experience in child welfare, family work, substance abuse, and mental health preferred.

Responsibilities

  • Meet one-on-one with residents to gather needs information and develop individual plans.
  • Collaborate with team to maintain building standards and perform wellness checks and inspections follow-up.
  • Design and implement daily activities aligned with Mercy Housing's program model.
  • Advocate for residents with public aid, social services, and other referrals as needed.
  • Develop leadership skills in residents and promote civic involvement.

Job description

The successful Case Manager II will work with the resident to develop appropriate case plans and goals and provide on-going assessment and support. Assists the resident to utilize community resources to meet individual needs. Works with residents to identify their skills and abilities to promote personal growth. Caseload is larger than that of a Case Manager I; serves as mentor to Case Manager I.

This is an on-site position.

Pay: $30-34

Duties
  • Meet one on one with residents to obtain specific needs information to create with residents a plan to achieve their goals.
  • Work with team to ensure building standards are met and participate in wellness checks and unit inspection follow-up.
  • Design and implement daily activities that promote Mercy Housing's program model.

  • Develop leadership skills in residents and resident involvement in civic life through programs.
Minimum Qualifications
  • Bachelor's Degree in Social Science or related field.

  • Experience in child welfare, family work, substance abuse, and mental health.
Preferred Qualifications
  • Master's Degree in Social Science or related field.
Knowledge And Skills
  • Strong organizational skills.
  • Basic math and analysis skills.
  • Basic computer skills (Microsoft Excel and Outlook); proficiency in Microsoft Word. Strong verbal, written, and communication skills. Ability to achieve expected results with residents.
  • Communicate effectively, both orally and in writing.
  • Demonstrate clear, professional boundaries; act in a professional manner.
