Job summary

Encompass Health is seeking a Case Manager II (Pool) to coordinate patient care from admission through post-discharge, guiding treatment plans with an interdisciplinary team. The role emphasizes timely discharge planning, documentation, and adherence to regulatory standards.

The qualified candidate will have CCM/ACM certification, nursing or allied health background, and rehabilitation experience. PRN availability offers flexible scheduling within a national inpatient rehabilitation leader.

Qualifications

• CCM® or ACM certification required OR must be obtained within two years of placement.
• RN licensure with BSN preferred; diploma acceptable in some states.
• 2 years of rehabilitation experience preferred.

Responsibilities

• Coordinate care with interdisciplinary team from admission to post-discharge.
• Establish tentative discharge plans and contingencies with the team.
• Complete case management addenda and required documentation.
• Monitor patient experience, quality, timeliness, and payor requirements.
• Attend ACT meetings to identify trends and reduce ACTs.
• Ensure accuracy of discharge and payor-related information in the patient record.
