Job summary

The Case Manager II at Harris County Sheriff’s Office provides comprehensive, individualized case management for an assigned caseload within the jail and community settings. You will conduct assessments, develop care plans, coordinate in-jail and community services, monitor progress, and support discharge planning.

You will work with correctional staff, medical and behavioral health professionals, courts, and community agencies to connect clients with needed supports while maintaining caseload

Qualifications

• Bachelor’s degree in a related field is required.
• Minimum of two years of case management or related experience.
• Experience conducting assessments and developing discharge plans.

Responsibilities

• Conduct screenings and assessments to identify needs and goals.
• Develop individualized care plans in collaboration with clients.
• Maintain assigned caseload and regular client contact.
• Coordinate in-jail and community services and resources.
• Support transition and discharge planning with clients and partners.
• Maintain timely case documentation and compliance with policies.
