Job summary

Nueces Center for Mental Health and Intellectual Disabilities in Corpus Christi, TX, seeks a Case Manager II for Jail CoC to ensure uninterrupted services during transitions into or out of the correctional setting and to help clients access LMHA services. Under supervision, you will screen and assess needs, develop treatment plans, coordinate care among agencies, and work with diverse stakeholders.

A Bachelor's degree is required; bilingual in English/Spanish preferred; valid Texas driver’s

Qualifications

• Bachelor's degree from an accredited college/university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, or related fields.
• Preferred: bilingual English/Spanish.
• Preferred experience with persons with serious mental illness (SMI) and justice-involved individuals.

Responsibilities

• Coordinate LMHA services and other supports during transition into or out of a correctional institution.
• Screen and assess the medical, psychiatric, or psychological needs of offenders with impairments and develop care plans.
• Collaborate with agencies and internal teams to ensure access to psychiatric, medical, social, vocational, and other services.
