Job summary

Telecare Corporation is seeking a Case Manager II - Mental Health to provide direct case management services for individuals with mental health and co-occurring disorders. This role helps clients live as independently as possible in their chosen community, under supervision.

Shifts are Monday–Friday, 8:30am–5:00pm, with an expected starting wage range of $23.00 per hour. Geographic differentials apply based on location; compensation reflects experience and local market.

Qualifications

• High School Graduation or GED and two years direct services in Mental Health or community services; or Associates and one year; or Bachelor's in Social Services.
• Valid and current driver's license and personal vehicle insurance with your name listed as a driver.
• Willingness to use personal vehicle to drive clients to appointments with IRS mileage reimbursement, and to use the company vehicle as needed.

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available:

Monday-Friday 830am-5:00pm

Expected starting wage range is $23.00

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.(weekly mileage reimbursement at the IRS rate)
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
