Job summary

Bridges to Change in Hillsboro, OR is hiring a full-time Case Manager I focused on gender-responsive support for female-identifying participants. You will deliver peer-led case management, document progress, and support recovery goals within a collaborative care team.

You will work 40 hours weekly, with some evenings or weekends as program needs, and operate in an environment upholding confidentiality, trauma-informed practices, and high professional standards.

Qualifications

• Must have Peer Wellness Specialist certification within 90 days of hire.
• Self-identifies as a peer with lived mental health/addiction-related experience relevant to the population served.
• Minimum 1 year of continuous sobriety for those with addiction-related lived experience.
• Background check clearance (ORCHARDS BCU) required.
• Valid driver’s license and vehicle with valid registration and insurance.
• Ability to conduct directly observed urinalysis with trauma-informed approach; female identity required as Bona Fide Occupational Qualification.

Responsibilities

• Deliver peer support and case management, dedicating at least 50% of time to direct client services.
• Support participants from diverse backgrounds and connect to services.
• Implement individual service plan goals.
• Promote community integration and recovery model exposure.
• Develop recovery skills, coping strategies, and independent living abilities.
• Provide encouragement and trauma-informed mentorship.
• Administer guideline-compliant random urinalysis screenings.
• Document services and progress notes within 24–72 hours.
• Participate in supervision, meetings, and trainings.
• Adhere to confidentiality and ethical standards.
