Job summary

The Connection in Middletown, CT is seeking a Case Manager 1 to support staff and deliver direct client care at Eddy Shelter. Responsibilities include intake assessments, development and implementation of individual care plans, and coordinating with treatment providers and community resources.

This non-exempt role reports to program management. Working within the Adult Services team, the Case Manager 1 assists in supporting clients’ recovery from mental illness, substance use, and homelessness,

Qualifications

• 2+ years related experience in the direct service and case management field
• Valid Connecticut Driver’s License
• Some Post-secondary education

Responsibilities

• Provide staff support and direct client care.
• Deliver comprehensive case management services including intake assessments and development/implementation of individual care plans.
• Coordinate with treatment providers and community resources.
• Support clients’ recovery within the Residential Behavioral Health Treatment care team.
