People Incorporated Mental Health Services is hiring Case Manager I/II for Targeted Case Management in Dakota County, MN. The role focuses on clients with chronic mental illness, developing person-centered plans and delivering services with autonomy.

The position offers full-time hours, a hybrid location in Dakota County, and a competitive hourly pay range ($20.75–$25.75) with a $1,000 hiring bonus for external candidates.

Qualifications

• Bachelor's Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing)
• RN with 3+ years combined training in psychiatry and community discharge planning in mental health
• Master's Degree in behavioral science or related field for Case Manager II

Responsibilities

• Perform functional assessments leading to individual plan development and access to services.
• Review plan effectiveness and adjust to maximize support.
• Assess client needs and ensure continuity of care.
• Refer clients to resources based on assessment and needs.
• Maintain daily records for continuity of service and funding reports.
• Document all interventions and interactions in the treatment plan.
• Attend client monthly meetings to monitor services.

Education

We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Hours:Full - Time, Monday - Friday, business hours

Location:Dakota County, Minnesota l Hybrid

Hiring Range:

• Case Manager I - $20.75 - $22.00Hourly, based on qualifications
• Case Manager II - $24.25 - $25.75 Hourly, based on qualifications

Hiring Bonus:$1,000 to external candidates

Main Job Duties:

• Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
• Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
• Assessing client needs and ensuring continuity of care.
• Making ongoing referrals to resources based on assessment and current client need.
• Maintaining necessary daily records for continuity of service and reports to funding agencies.
• Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
• Attending client monthly meetings to monitor services.

Requirements for All Targeted Case Manager I positions:

Education and Experience:

• Bachelors Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing from an accredited college or university)
• If no Bachelors Degree in a behavioral science or related field, the employee must meet one of the following:
• Have 3 or 4 years experience as a case manager aide; OR
• Be an RN without a Bachelors Degree and have 3 years or more combined specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.

Requirements for All Case Manager II positions:

• Valid driver’s license, excellent driving record and current auto insurance

Education and Experience Required for Targeted Case Manager II position:

• Bachelor’s Degree in behavioral science or related field
• 2,000 hours of supervised experience working with adults with severe and persistent mental illness
• Master’s Degree in behavioral science or related field.

OR

Preferred Qualifications for Targeted Case Manager II position:

• Experience providing direct services to adults with serious and persistent mental illness
• Experience with case management and commitment proceedings
• Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
