Volunteers of America Northern California & Northern Nevada, Inc. is seeking a full-time Case Manager I at the Mather Community Campus to serve singles program participants.

This role emphasizes case management, assessments, and accessing community resources to support housing stability. Under the Program Manager, you will coordinate referrals, manage service plans, document activities, and uphold policy and CARF standards while delivering compassionate assistance to residents and families in

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two years of successful experience in the delivery of human services.
• Knowledge of CPS and APS to support clients and families.
• Experience with homelessness or housing stability initiatives.
• Ability to maintain confidentiality and professional integrity.
• Strong communication, time-management, and data documentation skills.

Responsibilities

• Perform case management with program participants, including assessments and service plan development with housing goals.
• Review case plans weekly and adjust goals as needed.
• Lead group and individual sessions on life skills, housing, employment, and wellness.
• Coordinate referrals with community agencies to support participant needs.
• Document all contact, screenings, assessments, and reporting records.
• Ensure compliance with VOA policies and CARF standards; provide crisis intervention when needed.
