Adelphoi is seeking a dedicated Case Manager for the Family Finding program in Northumberland County, PA. This full-time role focuses on establishing a lifelong network of support for children and youth connected with Adelphoi's services.

Responsibilities include discovery, engagement, planning, and ongoing follow-up, with emphasis on safety, support networks, and documenting progress from Discovery to connections.

Qualifications

• BA/BS in Social Work or a related Human Service field.
• Minimum of 2 years of experience in the childcare field.

Responsabilités

• Facilitating casework sessions to assess for support, and safety needs of youth.
• Maintaining regular contact with the youth, their support resources and the referring agency.
• Documenting the process from Discovery to finding supports and connections.

What is Family Finding?

Family Finding establishes a lifetime network of support for children and youth who are disconnected or at risk of placement outside of their home and community. The Family Finding process identifies family members and supportive adults estranged from or unknown to the child especially those who are willing to become permanent connections for them.

What are the job responsibilities?

Case Managers are trained on and implement six clear goals and activities for any child/youth enrolled in the service: Discovery, Engagement, Planning, Decision Making, Evaluation, and Follow up on supports. Other responsibilities include:

• Searching for connections for the youth .

What are the requirements of the position?
