Job summary

Adelphoi USA, Inc. is seeking a Case Manager I for the Family Finding program in Northumberland County, PA. This full-time role focuses on identifying and linking youth with permanent supports and connections within their family and community.

Responsibilities include conducting casework sessions, assessing safety and support needs, maintaining regular contact with youth and referring agencies, and documenting the process from Discovery to connections.

Qualifications

• BA/BS in Social Work or a related Human Service field.
• Minimum of 2 years of experience in the childcare field.

What is Family Finding?

Family Finding establishes a lifetime network of support for children and youth who are disconnected or at risk of placement outside of their home and community. The Family Finding process identifies family members and supportive adults estranged from or unknown to the child especially those who are willing to become permanent connections for them.
