Job summary

The CM I focuses on efficient patient flow, safe transitions, and timely communications within the plan of care.

The role requires clinical RN or social work licensure with related discharge planning experience, strong analytical skills, and solid computer literacy.

Qualifications

• Registered Nurse with 1 year healthcare setting experience (Bachelor's degree in nursing within three years of hire).
• Bachelor's degree in social work or social services with licensure and 1 year of healthcare setting, discharge planning, care coordination, or OSF Case Management Internship.
• RN License (RN) or Social Worker Licensure (LSW/LBSW) or LPC in IL or MI.
• Solid computer skills, including proficiency with Microsoft software.
• Excellent interpersonal and written communication; ability to interact with diverse populations.

Responsibilities

• Collaborate with patient/family, physicians, and multidisciplinary team to develop a care plan from admission through discharge.
• Identify and manage patient flow to enhance continuity of care and length of stay management.
• Coordinate discharge planning and transitions to ensure patient safety and satisfaction.
