Job summary

ephraimmcdowell is seeking a Nurse Case Manager to coordinate patient care across the continuum. You will collaborate with physicians, nursing staff, patients, families, payers, and community resources to promote safe, efficient, high-quality, cost-effective care.

Qualifications

• Minimum of three (3) years of healthcare experience.
• Proficient computer and EMR skills.
• Knowledge of medical terminology, payer requirements, and medical necessity criteria.
• Strong organizational, critical thinking, communication, and interpersonal skills.
• Previous experience in case management, utilization review, discharge planning, or care coordination preferred.

Responsibilities

• Care coordination across providers, nursing staff, patients, families, payers, and community resources.
• Discharge planning assessments and transition-of-care management.
• Utilization management and length-of-stay oversight using payer guidelines.
• Documentation in the EMR and compliance with CMS/Medicare/Medicaid requirements.
• Participation in interdisciplinary rounds and care planning activities.
