Expected pay for this position is $33.85 - $39.83/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
POSITION SUMMARY:
The Case Manager (CM) I works in collaboration with the patient/family, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care and to independently develop a plan of care for each assigned patient from admission through discharge with minimal direction. Responsible for identifying, initiating and managing optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management. Utilizes their knowledge and expertise to evaluate the individual patient’s needs via comprehensive assessment and managing care coordination and discharge planning. The CM assesses and responds to patient/family needs by coordinating efforts of the interdisciplinary team members and identifies and resolves barriers that hinder effective patient progression of care. Ensures all communication is appropriate, accurate, timely, and consistent with the patient’s plan of care.
