Job summary

Socket.dev is seeking a Case Manager to provide comprehensive case management and intensive rehabilitation services for adults with severe mental illness in the Los Angeles area.

Responsibilities include linking clients to housing, medical and social services, delivering in-field services, and maintaining timely documentation while coordinating with the treatment team. The role requires strong collaboration, crisis intervention skills, and reliable transportation.

Qualifications

• Pass California DOJ background check and Live Scan.
• HS Diploma or AA degree in mental health or related field.
• Bachelor’s degree in social work, psychology or related field preferred.
• 4+ years of experience with HS Diploma.
• 2+ years of experience with AA degree.
• 1+ year of experience with BA degree; valid California driver’s license and car with adequate insurance.

Responsibilities

• Provide targeted case management and mental health rehabilitation services in the field.
• Collaborate with clinicians and staff to ensure consistent services.
• Conduct needs assessments and develop treatment plan components with clients.
• Involve clients in goal setting and empowerment opportunities.
• Meet assigned clients per treatment plan frequency and track participation.
• Deliver individual and group mental health rehabilitation and related skills training.
• Assist clients in linking housing, benefits, health services, and transportation.
• Monitor medication adherence and coordinate with medication support.
• Provide crisis intervention to prevent hospitalization or incarceration; participate in safety planning.
• Maintain contact with clients’ families as appropriate.
• Maintain 6 hours of billable service per day and 48-hour documentation in EHR.
• Prepare for weekly clinical staff meetings.
• Other duties as assigned.

Job description

Under the direction of the Program Manager, the Case Manager provides comprehensive and intensive case management services to adults with severe and persistent mental illness. Case management services include linkage to community resources such as housing, medical, educational and vocational services. In addition, provide rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports clients in meeting their goals. Adheres to all required documentation standards.

Position Requirements:

• Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
• High School Diploma or Associates Degree in mental health or related field.
• Bachelor’s Degree in Social Work, Psychology or related field preferred.
• Must have at least four years of experience working in the field of mental health / social services with a HS Diploma.
• Must have at least two years of experience with an AA degree in the field of mental health / social services.
• Must have at least one years of experience with a BA degree in the field of mental health / social services. Must have valid California Driver's license and the availability of a car with adequate insurance.

• Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation.
• Knowledge of Los Angeles County Department of Mental Health's Rehab Option model of service provision and documentation requirements.
• Ability to work as a team member and to work with other agencies, community resources and client’s families.

Primary Duties:

• Provide targeted case management and mental health rehabilitation services to clients, linking them with all appropriate community services. Services are conducted in the field.
• Work as a team with clinician, psychiatrist and other inter/intra agency staff to provide consistent services.
• Conduct needs assessment with the client in the beginning treatment and on an ongoing basis when necessary to identify rehabilitation and linkage needs of the client. Collaborate with clinician and client to identify areas needed to be included in the treatment plan.
• Ensure clients are involved in establishing goals impacting their functional impairments. Involve clients in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible.
• Meet with all assigned clients based on frequency stated in the client’s treatment plan. Monitor client’s participation in services.
• Conduct mental health rehabilitation and recovery oriented services both individually and in groups as necessary. Provide mental health rehabilitation services, such as, counseling, money management, social skills training, and other independent living skills as needed by assigned clients.
• Collaborate with client and treatment team as necessary to assist clients in linkage to community resources including but not limited to: housing resources, obtaining benefits, obtaining basic necessities, health services, and linkage to transportation.
• Track client’s medication to ensure that they are medication compliant and collaborate with medication support team to ensure ongoing compliance and attendance to doctor’s appointments.
• Provide crisis intervention to clients at risk of hospitalization and /or incarceration in an effort to avert hospitalization and /or incarceration. Participate in safety planning with clients to ensure clients have knowledge and access to crisis intervention services after hours.
• Maintain contact with client’s family and/or significant other support persons as appropriate.
• Maintain productivity of 6 hours of billable service per day and submit progress notes in electronic health records within 48 hours of direct services. Documentation is to be completed and submitted accurately and in a timely manner. The services delivered are to be a documented in a manner consistent with established standards for the assigned program.
• Prepare for weekly clinical staff meetings by having current information on the status of all assigned clients
