Job summary

Comunilife Inc. seeks experienced Case Managers for the Dona Rosita I Program in the Bronx. You will assess resident needs, create case plans, and link residents to housing, health, and community services, both inside the residence and in the community.

You will collaborate with Dona Rosita Program staff to support independent living skills and permanent housing placement, working with residents and families within a congregate housing setting that includes daily meals and activities.

Qualifications

• Bachelor’s Degree in human services or related field with two (2) years’ case management work experience; Associates Degree with four (4) years; High School Diploma with six (6) years’ experience will be considered.
• Knowledge of HASA Congregate Housing Program guidelines.
• Direct service experience with persons living with HIV/AIDS, substance abuse, serious mental illness, and homelessness.
• 5 Day/35 work week Non-Remote Position (In-Office/Some Field Work).
• Excellent oral and written communication skills.
• Proficient in Google Workspace and Microsoft Office Suite.

Responsibilities

• Coordinate program admission process with HASA Case Management Unit and Housing Unit.
• Conduct interview of referrals within 24 hours of the HASA Referral.
• Arrange and coordinate the move of accepted residents as determined by HASA and the program.
• Monitor and assist the residents with problems at local entitlement offices (Social Security, Medicaid, income support, etc.).
• Establish permanent housing referral network and link residents with health providers and support systems.
• Institute referrals and act as liaison/advocate to ensure receipt of required services.
• Provide individual and group counseling on topics such as substance abuse, money management, and HIV/AIDS information.
• Document all direct activities and referrals in resident records and provide monthly caseload updates to Clinical Supervisor.
• Attend mandated meetings and in-service training; assist residents in gaining independent living skills and permanent housing placement.

Comunilife is looking for experienced Case Managers to fill open positions for their Dona Rosita I Program in the Bronx. The Case Manager is responsible to assess the resident's needs and develop case plan to ensure that resident is linked to needed services both inside the residence and in the community. Assist resident in developing necessary skills to live more independently in permanent housing. Responsible for general case management, working with resident’s families and significant others for Residence, which provides congregate treatment services, i.e., one (1) meal a day; case management and recreational activities to persons living with HIV/AIDS. Other responsibilities include working harmoniously with Dona Rosita Program staff in accordance with Comunilife principles and guidelines. Document all direct activities including resident served case closures and direct service advocacy interventions.Document all contacts and referral activity in resident records.When necessary and appropriate, accompany residents in medical appointments and/or arrange ambulate services.Attend all mandated meetings and in-service training.Assist resident in gaining the skills necessary to live more independently and arrange for placement in a permanent housing program.Establish working relationship with HASA case management staff to ensure quality client care.Participate in ongoing evaluation process to assess direct service activities for program modification.
