Job summary

Samaritan Daytop Village, Inc. is seeking a full-time Case Manager to work with clients experiencing homelessness in Jamaica, Queens.

The role involves interviewing clients, assessing needs, and collaboratively developing Independent Living Plans, coordinating housing referrals, and advocating for access to healthcare, education, and legal services. You will maintain detailed records, monitor progress, and liaise with community partners.

Qualifications

• Experience in case management or social services
• Ability to document progress and coordinate with external partners
• Commitment to client confidentiality and professional ethics
• Knowledge of NYC housing, healthcare, legal, childcare, and educational resources
• Strong written and verbal communication skills
• Ability to manage multiple responsibilities and meet reporting deadlines

Responsibilities

• Build relationships with clients and maintain confidentiality
• Review eligibility documents and explore non-shelter options
• Create and manage client files and records
• Conduct intake interviews and psychosocial assessments
• Develop and update Independent Living Plans with goals
• Refer clients to housing and community services
• Advocate for clients and remove barriers to stability
• Track progress and document updates
• Follow up with clients and referral agencies
• Provide data for reports (weekly, monthly, quarterly, annual)
• Coordinate with outside organizations for services
