Qualifications

• High School Diploma or equivalent required.
• Associate or Bachelor’s Degree in Human Services, Social Work, or related field strongly preferred.
• 1+ year of case management or social services experience with homeless populations.
• Strong interpersonal and communication skills for client interviews and reporting.
• Ability to maintain accurate records and meet reporting deadlines.
• Familiarity with NYC housing, healthcare, legal, childcare, and educational resources.

Responsibilities

• Build professional relationships with clients and maintain confidentiality.
• Review client eligibility documents and explore non-shelter alternatives.
• Create and manage client files and records.
• Conduct intake interviews and psychosocial assessments.
• Develop and update Independent Living Plans with goals.
• Refer clients to permanent housing and community services.
• Advocate for clients to overcome barriers and monitor progress.
• Document updates and client outcomes regularly.
• Liaise with outside organizations for various services.
