Job summary

Felton Institute is seeking a Part-Time Case Manager for the Bridge Housing site in Santa Rosa. You will provide care, case management, and crisis intervention within a multi-disciplinary team serving individuals with serious behavioral health conditions and homelessness.

The role emphasizes evidence-based practices, client-centered support, and collaboration with community partners, with a 24-hour operation focus and on-site work schedule.

Qualifications

• 2+ years of case management, SUD and mental health treatment experience for TAY and adults (incl. co-occurring disorders)
• Bachelor’s degree or higher in Social Work, Counseling, Psychology, or related field—or currently enrolled
• Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills
• Knowledge of de-escalation, mitigation and pre-escalation techniques
• Willingness to work from a strength-based, harm reduction, client-centered model

Responsibilities

• Provide care and case management, mental health and substance use recovery and wellness services, and crisis prevention and intervention within a multi-disciplinary team
• Utilize evidence-informed and evidence-based interventions to address guests' needs and goals
• Collaborate with community partners acting as access and intervention points
• Meet documentation and deadlines; comply with Medi-Cal requirements
• Perform administrative tasks (chart completion, meeting attendance)
• Connect guests with short and long-term housing, transportation vouchers, emergency food, and hygiene products
• Assist guests in applying for public benefits
• Link guests to primary care and health care services
• Reduce hospitalization, homelessness and incarceration; assist re-integration into the community
• Develop gender- and culturally-responsive curriculum
• Involve food prep as needed
• Develop trainings for guests on DV, abuse, loss of identity, substance use, depression, financial hardship and housing displacement
