Job summary

Community Assisted & Supported Living Inc. seeks a case manager to provide direct support to adults with mental health needs. You will help with daily living activities, skill-building, and housing access, while coordinating transportation and community resources.

You’ll document progress in EMR, participate in monthly housing checklists, and ensure compliance with safety policies. This is a full-time on-site role in Sarasota, FL with benefits.

Qualifications

• Bachelor's degree from an accredited university in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or related field.
• Minimum of three years full-time or equivalent experience with adults with severe and persistent mental illness OR five years with adults with persistent mental health diagnosis.
• Knowledge of state and federal statutes, rules and policies affecting the target population.

Responsibilities

• Provide direct assistance for daily living activities and personal goals.
• Develop and coach skills per the service plan.
• Coordinate transportation for medical and other appointments.
• Develop weekly activity schedules aligned with client needs.
• Ensure safety and report incidents per agency procedures.
• Assist with community integration and outreach activities.
• Document progress in EMR within 48 hours of service.
• Maintain housing and housing readiness for residents.
• Meet with clients regularly to address barriers to self-sufficiency.
• Assist clients to apply for and maintain benefits.
• Conduct monthly house meetings and client outreach.
