Horizons Kingman County is seeking a part-time Case Manager to deliver interactive mental health services to adults and children in the community. You will work with the treatment team to develop client-centered plans and help clients understand symptoms and solutions.

The role emphasizes a strengths-based, skill-building approach and collaboration with families, providers, and community partners. A strong focus on privacy and professional communication is essential.

Qualifications

• Bachelor's degree preferred in Social Work, Psychology, or related field.
• Associate's degree with 2+ years experience in educating and teaching clients about self-care, daily living activities.
• Total combined education and experience of 4+ years in related work.

Case Manager - Horizons Kingman Co. - PT Variable Days/Hours

Essential Responsibilities:

• Provide skill training on target goals and objectives with measurable outcomes, with measurements as determined by medical necessity by the Qualified Mental Health Professional
• Deliver intensive, community-based, interactive mental health services to children and adults utilizing a strengths-based, skill-building philosophy
• Participate in the development and maintenance of a client-centered Treatment Plan along with the family and treatment team
• Assist the youth, or adult, develop a basic understanding of their symptoms and possible solutions
• Assist the adult, or youth, and family to enlist natural community supports in developing solutions
• Assist in coordination of mental/physical healthcare services and facilitate communication between the client, service providers, family and community partners
• Act in accordance with the established mission, vision, and values.
• Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.

Minimum Qualifications :

Required Education and Experience

• Bachelor's degree, preferably in Social Work, Psychology, or some related field
• Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
• Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years

Required License/Certifications/Registrations

• CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
• Completion of Adult Case Management training within 60 days of employment.
• Valid driver's license, insurance, and a dependable vehicle if position requires driving.

Preferred Qualifications:

Preferred Education and Experience

• Bachelor's degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
• Three to five years' professional experience in mental health services, crisis response, or a related field.

Knowledge, Skills, and Abilities :

• Familiarity with community mental health services.
• Strong organizational skills
• Intermediate proficiency with Microsoft Office
