Job summary

Lifting Up Westchester is seeking an experienced case manager to support shelter clients and residents. You will develop and coordinate service plans for a full caseload, coordinate referrals, and maintain comprehensive client records to promote housing stability.

You will engage vulnerably housed adults using motivational interviewing and empathetic support, collaborating with internal programs and outside resources to connect clients to essential services.

Qualifications

• Bachelor's degree or higher in human services or related field and/or at least 4 years of proven Human Service work experience.
• Two years of experience working with homeless, mentally ill, indigent, chemically addicted adults preferable.
• Experience working with D.S.S. and other service providers.
• Must have valid driver's license.
• CASAC or CASAC-In-Training or equivalent experience preferred.
• Excellent organizational, time management and writing skills.
• Must be dependable, flexible, and disciplined, and possess sensitivity, tact, and resourcefulness.
• Bi-lingual an asset.

Job description

Summary

Seeking a professional, experienced case manager familiar with shelters and housing. Candidate will work with vulnerable, indigent male and female adults, develop and coordinate service plans for full caseload assigned including resident and drop in population ; utilize day to day working relationship with internal programs, outside agencies and resources relevant to client population; make appropriate referrals and follow-up on referrals; participate in weekly one on one supervision, case conferences; organize special didactic activities for clients; follow-up on special problems of individual clients; maintain and regularly update client records; and submit program reports as required

Engage and refer to local treatment providers serving chronically homeless individuals suffering from disorders of mental illness and substance abuse. Utilizing trust-building, motivational interviewing, and integrated treatment methodologies, monitor progress and make referrals to additional support services focused on helping clients achieve housing stability.

Job Requirements
