Job summary

All American Home Care is seeking a Case Manager – Home Care to coordinate and monitor services for an active caseload of 80–100+ patients. You will ensure high-quality care, effective scheduling, and compliant authorizations while communicating with patients, families, and referral sources.

The ideal candidate has 2–3 years of experience in home care or case management, strong organizational and communication skills, and is comfortable working in a fast-paced environment.

Qualifications

  • High school diploma or equivalent.
  • 2–3 years of experience in home care, case management, or service coordination.
  • Experience managing high-volume caseloads in a fast-paced environment.
  • Experience with scheduling, staffing coverage, and authorization management.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Manage and oversee a caseload of 80–100+ patients.
  • Develop and evaluate individualized home care plans with patients and families.
  • Conduct routine follow-up calls to assess satisfaction and address concerns.
  • Review and break down payer authorizations to ensure services stay within approved hours.
  • Monitor utilization of authorized service hours and adjust as needed.
  • Schedule home health aides and maintain full shift coverage.
  • Build and maintain an on-call pool of caregivers.
  • Match patient needs with caregivers based on skills and availability.
  • Communicate with referral sources, case managers, and payers to support patient care and growth.
  • Resolve grievances and document concerns per policy.
  • Monitor caregiver performance and document issues.
  • Maintain accurate patient and personnel documentation per regulations.

Job Title: Case Manager – Home Care
Job Summary

The Case Manager is responsible for coordinating, monitoring, and evaluating home care services for an active caseload of 80–100+ patients. This role ensures high-quality care delivery, effective scheduling, authorization compliance, and strong communication with patients, families, caregivers, referral sources, and internal teams. The ideal candidate thrives in a fast-paced home care environment, demonstrates strong leadership and organizational skills, and is committed to delivering outstanding patient and caregiver experiences while supporting departmental and company goals.

Key Responsibilities
  • Manage and oversee a caseload of 80–100+ patients, ensuring continuity, quality, and retention of care.

  • Develop, monitor, and evaluate individualized home care plans in collaboration with patients, families, and caregivers.

  • Conduct routine follow-up calls with patients and/or family members to assess satisfaction, address concerns, and resolve issues promptly.

  • Review, interpret, and break down payer authorizations, ensuring services are delivered within approved hours and guidelines.

  • Monitor and address overutilization or underutilization of authorized service hours.

  • Schedule home health aides and ensure full shift coverage, including managing last-minute call-outs and securing appropriate replacements.

  • Build and maintain an on-call pool of caregivers in coordination with HR and recruitment teams.

  • Match patient needs with suitable caregivers based on skills, availability, and compatibility.

  • Communicate effectively with referral sources, case managers, and payers to support patient care and business growth.

  • Resolve and document grievances or complaints from patients, families, and caregivers.

  • Monitor caregiver performance, document personnel issues, and report concerns in accordance with company policy.

  • Maintain accurate and up-to-date patient and personnel documentation in compliance with State and Federal regulations.

  • Promote caregiver engagement, morale, and retention through strong communication and support.

  • Support team initiatives and contribute to departmental performance and financial goals.

  • Ensure compliance with company policies, procedures, licensing requirements, and quality assurance standards.

Qualifications
  • High school diploma or equivalent required.

  • Minimum 2–3 years of experience in home care, case management, or service coordination.

  • Demonstrated ability to manage high-volume caseloads in a fast-paced environment.

  • Experience with scheduling, staffing coverage, and authorization management.

  • Strong written and verbal communication skills.

  • Excellent organizational, problem-solving, and time-management abilities.

  • Ability to maintain confidentiality and handle sensitive information.

  • Proficiency with computer systems and electronic documentation.

  • Experience with HHA Exchange and EVV systems preferred.

  • Bilingual English/Spanish preferred.

  • Emotional and mental maturity to establish and maintain effective working relationships with patients, families, caregivers, referral sources, and administrative staff.
