Job summary

The Salvation Army of the Twin Cities is seeking a Rapid Re-Housing (RRH) Case Manager to develop, implement, and oversee the RRH program. You will empower participants to navigate housing resources, complete Coordinated Entry assessments, and collaborate with community partners to secure and maintain housing.

Responsibilities include holistic, strength-based case management, documentation, and engagement with internal and external stakeholders to advance housing stability and self-sufficiency

Qualifications

• Bachelor’s degree or equivalent required; social services experience preferred.
• If degree is in social work, must be licensed by the state board.
• Minimum experience in social services and case management is required.
• Background checks, valid driver’s license, and MVR compliance required.

Position Summary

The Salvation Army of the Twin Cities is dedicated to “Doing the Most Good” by providing holistic, person-centered services that empower individuals and families. Core programs include case management, basic needs assistance, housing and shelter services, community resource connections, and pastoral care.

HOPE Harbor and Harvest Hills provide supportive housing and wraparound services that help individuals transition from crisis to stability, with a focus on building self-sufficiency, resilience, and community.

The Rapid Re-Housing (RRH) Case Manager is responsible for assisting in the development, implementation, and oversight of the RRH program. The Case Manager empowers participants in navigating housing and community resources through targeted case management services. The Coordinator empowers participants experiencing homelessness and housing instability to secure and maintain housing, increase their self-sufficiency, and connect to their communities. Additionally, the Coordinator provides direct assistance and supports Coordinated Entry efforts by completing participant assessments and collaborating with community partners.

$20.81 to $25.35 per hour, 35 hours a week

Essential Functions (approximate time spent)

• Housing Placement and Community Partnerships (40%): Collaborate with community partners to coordinate referrals and housing placements. Develop and maintain relationships with community resources and service providers, complete intake for eligible RRH participants, and promote RRH and The Salvation Army social services throughout the community.
• Case Management and Participant Support (45%): Provide strength-based, person-centered, holistic case management to RRH participants, including completing Coordinated Entry assessments, developing individualized housing stability plans, coordinating services, and supporting participants in achieving program goals.
• Documentation and Data Management (10%): Maintain accurate, timely participant case files and complete required database entry and program documentation in accordance with organizational and funding requirements.
• Program and Community Engagement (5%): Participate in internal and external meetings, trainings, and community activities that support Salvation Army and RRH goals, partnerships, and program objectives.

Education and Experience

• Bachelor’s degree in a relevant field. An equivalent combination of education and experience may be considered in lieu of a degree.
• Must be licensed by the state board if the degree is in social work.
• Previous experience in social services required.
• Experience providing long-term case management preferred.
• Must pass background checks, possess a valid driver’s license, and meet MVR requirements.
