Job summary

Catholic Charities of the Diocese of Albany is seeking a Case Manager to support individuals in navigating services and resources, with a focus on wellness and housing stability. The role involves intake, assessment, and ongoing case planning.

You will coordinate ISP plans, track progress, and ensure timely documentation. Hybrid benefits and strong team support accompany a 40-hour workweek in Albany or Troy.

Qualifications

• Associate's Degree in Social Work or Bachelor’s preferred.
• 1–3 years’ experience in Human Services or related field.
• Bilingual Spanish preferred.

Responsibilities

• Complete timely and accurate documentation in the Uniform Case Record (referral intake packets, related intake information, referrals, entitlement documentation, logbooks).
• Review intake information for new referrals and perform initial screening assessments; make recommendations on acceptance/denial.
• Perform housing office intake assessments for new tenants and annually thereafter.
• Develop and implement the ISP and report outcomes every 6 months; adjust as needed.
• Address monthly progress against housing and service milestones.

The Case Manager plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability.

In this role you will:

• Complete the timely and accurate documentation in the Uniform Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, logbooks, and other areas as assigned.
• Review all intake information for new referrals and performs initial screening assessments for intake criteria and makes recommendations regarding acceptance or denial.
• Perform Catholic Charities Housing Office intake assessments of each new tenant and then annually thereafter.
• Develop and implement the ISP and then every 6 months thereafter, reports outcomes and makes needed changes.
• Address the monthly progress achieved against specific milestones and targets related to housing and referral for services.
• Case ManagerSalary: $ 21.50
• Case Manager Schedule: Monday - Friday (35 & 40 hours per week)
• Case Manager Location: Hiring for both Albany ( DePaul) & Troy (St. Peters)
• No subject-to-call shifts required
  

Qualifications:

• Education:
  • Associate's Degree in Social Work, Bachelor's preferred
• Experience:
  • 1-3 years' experience with relevant experience in Human Services or a related field
  • Bilingual / Spanish preferred
• Valid Driver's License along with car insurance, registration, and access to working vehicle required
