Job summary

Asana Recovery is seeking an experienced Case Manager for our outpatient clinical team in Fountain Valley, CA. This weekend, Saturday and Sunday position focuses on supporting patients throughout outpatient treatment.

You will perform intake assessments, discharge planning, crisis intervention, and individual/group sessions, with documentation in Kipu. A CADC/CATC/SUDCC certification and 1 year of CM experience are required.

Qualifications

• Current CADC/CATC/SUDCC certification required.
• At least 1 year of case management experience with strong references.
• Advanced knowledge of the 12-step model and its residential application.
• Knowledge of dual diagnosis treatment and non-pharmacological management.
• Strong assessment, discharge planning, and documentation skills.

Responsibilities

• Conduct intake assessments and support discharge planning.
• Provide crisis intervention and individual sessions.
• Facilitate group sessions and job coaching as part of treatment plan.
• Document activities in the treatment system (Kipu).

Asana Recovery, named as one of Newsweek’s “America’s Best Addiction Treatment Centers” in 2022, is looking to add an experienced Case Manager to our outpatient clinical team.

• Weekend Position (Saturday & Sunday)
• Must have valid Drivers License with No Restrictions

SUMMARY

The role of the Case Manager is crucial for ensuring the success of a patient while in outpatient treatment.

The Case Manager is ultimately responsible for the support and mentoring of their patients throughout their outpatient stay. Daily responsibilities will vary from doing intake assessments, discharge planning, crisis intervention, one-on-one sessions, group facilitation, job coaching, and other tasks to support the patient’s treatment plan. Documentation of these activities in Kipu will be required.

REQUIREMENTS

• Current certification through CADC/CATC/SUDCC as a Certified Alcohol and Drug Counselor
• One year of experience as a Case Manager with strong professional references attesting to a high degree of clinical sophistication
• Advanced knowledge of the 12-step treatment model and its application in residential treatment
• Specific knowledge is helpful in process addiction and treatment resources
• Advanced assessment, treatment planning, discharge planning, and documentation skills
• Knowledge of treatment and referral communities in Southern California
• Knowledge of dual diagnosis treatment practices including non-pharmacological symptom management techniques
• Current certification in First Aid/CPR
