The position offers a weekday schedule, a $8,000 sign‑on bonus, and a comprehensive benefits package. Local travel is required, and eligibility for the long-term care functional screener certification is preferred.

Competenze

• Bachelor’s Degree in a Human Services or closely related area required.
• 3 years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• Eligible for long-term care functional screener certification.
• Regular local travel with valid driver's license and required insurance.

Mansioni

• Manage an assigned caseload in collaboration with an RN Case Manager.
• Complete comprehensive member assessments, care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in adherence to established standards.
