Job summary

DISC Village is seeking a care coordination professional to engage families and coordinate services across multiple sites, including homes and schools. The role focuses on developing culturally sensitive family plans with measurable objectives and strong team involvement to support parents in achieving outcomes.

The position requires a Bachelor’s degree in a related field and experience with adults with behavioral health needs, plus a Florida driver’s license.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or related field.
• Minimum of one year of experience working with adults with behavioral health needs and child welfare involvement.
• Bachelor’s or Master’s degree with a major in another field and a minimum of three years of experience working with adults with substance use disorders.
• Must have a valid Florida driver’s license and applicable state required car insurance.

Responsibilities

• Coordinate outreach to engage parents and potential team members in care coordination services.
• Identify immediate and/or potential crises and safety concerns and develop a plan that addresses these concerns and prevents future crises.
• Develop a comprehensive family care plan with measurable objectives and action steps focusing on strengths, needs and culture, with team involvement.
• Link parents with services and educate them on parental rights and ensuring integration of health care across systems.
• Support and utilize community networks and coalitions to enhance communication and collaboration in service planning and delivery.
• Travel required between offices, and other sites including homes, schools and community locations where clients are served.
