Job summary

Comunilife is seeking a Case Manager for its Bronx residential program. You will develop supportive relationships with clients, link them to community-based medical and mental health care, provide crisis intervention, and assist access to primary care to promote independent living.

The role requires a Bachelor’s in human services, at least two years’ experience, and familiarity with HIV/AIDS and mental health challenges.

Qualifications

• Bachelor's Degree in human services or related field required.
• Two years of work experience.
• Experience with HIV/AIDS populations, chronic mental illness, or substance use disorders is a plus.
• Bilingual English/Spanish is a plus.
• Proficiency with Microsoft Office and Google product suites.

Responsibilities

• Completing comprehensive assessments, service plans, progress notes
• Monitoring and reassessing clients’ needs for referrals and services
• Providing psychoeducation on relevant topics such as substance use disorders, entitlements, etc.
• Providing individual and group workshops
• Working collaboratively in an interdisciplinary team to ensure comprehensive, client-centered services are delivered and documented.

Comunilife Inc is looking to fill a Case Manager position for its Bronx residential program. The Case Manager develops supportive relationships with clients; links them to community‑based medical and mental health care, and provides crisis intervention as needed. The Case Manager provides support to access primary medical care and other necessary services to increase clients’ capacity for independent living, monitors progress, and provides referrals as needed. Essential Duties

• Completing comprehensive assessments, service plans, progress notes
• Monitoring and reassessing clients’ needs for referrals and services
• Providing psychoeducation on relevant topics such as substance use disorders, entitlements, etc.

• Working collaboratively in an interdisciplinary team to ensure comprehensive, client‑centered services are delivered and documented.

Qualifications

• Bachelor's Degree in human services or related field; Credentialed Alcohol and Substance Abuse Counseling (CASAC‑T or CASAC) is a plus
• Two (2) years’ work experience
• Experience with HIV/AIDS‑afflicted individuals, chronic mental illness, substance abuse individuals and their families is a plus
• Bilingual skills in English/Spanish is a plus
• Knowledge and hands‑on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness
• Excellent oral and written communication skills; proficiency with Microsoft Office and Google product suites
• Ability to perform and manage tasks in a team approach; excellent interpersonal skills
• Ability to handle complex challenges and balance multiple demands on time and attention
• Ability and willingness to perform all duties as workload necessitates
