Job summary

PermiaCare is seeking a Case Manager to support individuals with mental health needs across multiple counties in Texas. You will manage a caseload, coordinate services, and maintain precise documentation to meet program requirements.

Applicants should hold a relevant Bachelor's degree and pursue QMHP certification within six months. A Texas driver’s license and driving record acceptable to the agency are required.

Qualifications

• Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, or related fields.
• Or a bachelor's with 30+ credits in the listed fields.
• QMHP certification must be obtained within 6 months.

Responsibilities

• Case managers maintain an assigned caseload and provide required services.
• Maintain accurate records and documentation within required timeframes.
• Meet unit performance measures and targets to ensure clients receive needed services.

Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, early childhood education or early childhood intervention
