Job summary

Telecare is seeking a Direct Care case manager in San Diego to work full-time on-site. Under supervision, you will support individuals with mental illness to live independently in their community and share expertise with the treatment team.

The role requires a high school diploma or equivalent (or higher), valid driver's license, and driving clients to appointments. Compensation starts at $23–$26.25 per hour, with geographic pay differentials applied.

Qualifications

• High School Graduation or GED and 3 years in mental health or community services; or Associate’s with 2 years; or Bachelor’s in Social Services.
• At least 1 year of experience in the area of specialty served (e.g., Benefits, Education, Employment).
• Valid driver’s license and personal vehicle insurance; willing to use personal vehicle for client transportation.

Responsibilities

• Provide primary support and case management to individuals with mental illness to promote independent living.
• Contribute therapeutic knowledge to the treatment team and program.
• Schedule: Full-Time, 8:00 AM - 4:30 PM, Monday–Friday.

What You Will Do to Change Lives

Under direct supervision, serves as a primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community. In addition, this position provides knowledge of a specialty area to the treatment team and program.

Schedule: Full-Time | 8:00 AM - 4:30 PM | Monday - Friday

Compensation: Expected starting wage range is $23 - $26.25.

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
• Must possess at least 1 year of experience in the area of specialty served (e.g. Benefits, Educational, Employment, etc.)
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
