Summary


The Downtown Women’s Center seeks a full-time Case Manager to provide outreach, housing placement, and individualized case management services to individual women and women with children who are homeless due to domestic violence. The Case Manager will be integral in developing outreach plans to engage women in services, establish eligibility, complete enrollments into the program and connect clients to housing resources. Once a client is enrolled into the program, this position ensures clients have the needed financial resources to secure and maintain permanent housing, provides referrals, conducts safety plans, and assists with other services as needed. This position is part of an interdisciplinary team and provides innovative services that are at the forefront of homeless services. An ideal candidate practices Trauma-Informed Care and approaches case management with a strengths-based approach. This position is a member of the Clinical Department and reports directly to the DV Sr. Clinician.



Essential Functions

  • Provide individualized case management services to clients (on-site and in the field); create individual service plans that ensure housing stability, address each clients’ needs and goals, identify and provide needed resources and referrals.
  • Assist clients in securing housing, housing related assistance (i.e. furniture, utility assistance, etc.), basic needs, medical and mental health services including substance abuse treatment when necessary, job training access, and public benefits where eligible.
  • Work to effectively meet clients’ needs and resolve individual barriers through follow-up, advocacy, and collaboration with DWC staff and other community service providers.
  • Maintain accurate and timely data records of activities, case management notes, and financial assistance and other services provided to each client.
  • Provide crisis intervention and group facilitation as necessary.
  • Participate in individual and group supervision, trainings, and meetings.
  • Utilize Evidence Based Practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First practices.
  • Other duties as assigned to support the mission of the organization.

Supervisory Responsibility


This position does not have supervisory responsibilities.


This job operates in a professional office environment and in the community, including where clients reside. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary. The employee must be able to operate a vehicle.


Position Type and Expected Hours of Work
This is a full-time position. Days and hours of work are 8:30am-5pm.


Travel
Regular travel throughout Los Angeles County is required in this position.


Required Education and Experience

  • Bachelor’s degree in social work or related field and strong knowledge of the complexity of poverty and women’s homelessness, domestic violence, mental illness, trauma, substance abuse, health issues, and older adults preferred.
  • Two years of experience working as a case manager/housing specialist/similar position and knowledge of community resources preferred
  • At least two years of experience providing domestic violence services as a case manager or similar preferred
Required Knowledge/Skills/Position Qualifications
  • Excellent active listening and crisis intervention skills with ability to exercise and model good judgment.
  • Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences.
  • Ability to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with effective record keeping.
  • Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required
  • HMIS or other database management preferred.
  • 40-Hr Domestic Violence Certification a plus.
  • Current valid California state driver’s license, insurance, and vehicle with good driving record required.
  • Oral and written fluency in Spanish and English a plus.

Work Authorization/Security Clearance (if applicable)
  • Criminal background check and clearance required
