JOB DESCRIPTION

Job Summary

Molina Healthcare Services (HCS) collaborates with members, providers, and multidisciplinary teams to assess, facilitate, plan, and coordinate integrated care across the continuum, including behavioral health and long-term care, for members with high-need potential. HCS staff aim to ensure patients progress toward desired outcomes with quality, medically appropriate, and cost-effective care based on illness severity and service site.

This position supports our Washington State Plan. We seek candidates with LCSW, LMSW, or LMFT licensure. Experience with motivational interviewing and working with members with co-morbid conditions is preferred. Knowledge of and experience supporting behavioral health conditions are highly valued. Work schedule: Monday - Friday, 8:00 AM to 5:00 PM PST.

This is a remote position with field travel within Clark, Cowlitz, Multnomah counties, and/or within 25 miles of Vancouver, WA.

Required licensure: WA LCSW, LMSW, or LMFT.

KNOWLEDGE/SKILLS/ABILITIES

  • Complete clinical assessments of members within regulated timelines, determining eligibility for case management based on clinical judgment, health changes, psychosocial wellness, and assessment triggers.
  • Develop and implement case management plans collaboratively with members, caregivers, healthcare professionals, and support networks to address needs and goals.
  • Conduct telephonic, face-to-face, or home visits as needed.
  • Monitor care plans regularly to evaluate effectiveness, document interventions, and suggest modifications.
  • Maintain an ongoing member caseload for outreach and management.
  • Promote integrated services, including behavioral health and long-term supports, to enhance care continuity.
  • Implement specific Molina wellness programs, such as asthma and depression management.
  • Facilitate interdisciplinary care team meetings and informal collaboration.
  • Utilize motivational interviewing and Molina's clinical guideposts to educate, support, and motivate members.
  • Assess for barriers to care, provide coordination, and assist members in addressing concerns.
  • Collaborate with RN case managers and supervisors as needed.
  • Behavioral health and social science case managers may provide consultation and resources to peers.
  • Local travel of up to 40% may be required, depending on case complexity, state regulations, and position location within Molina’s Central Programs.

JOB QUALIFICATIONS

REQUIRED EDUCATION:

Completion of an accredited LVN or LPN program, or a Bachelor's or Master's degree in social sciences, psychology, gerontology, public health, social work, or related fields.

REQUIRED EXPERIENCE:

1-3 years in case management, disease management, managed care, or medical/behavioral health settings.

REQUIRED LICENSE, CERTIFICATION, AND ASSOCIATION:

Active, unrestricted license in good standing. Valid driver’s license with a good record and reliable transportation to drive within the applicable area.

STATE-SPECIFIC REQUIREMENTS:

For roles serving Wisconsin’s Family Care programs: a Bachelor's degree and at least one year of professional experience.

PREFERRED EXPERIENCE:

3-5 years in case management, disease management, managed care, or medical/behavioral health settings.

PREFERRED LICENSE, CERTIFICATION, AND ASSOCIATION:

Licenses or certifications such as LCSW, APSW, CCM, CHEP, LPC/LPCC, Respiratory Therapist, or LMFT.
